Please reference the linked file prior to developing or submitting exclusivity waiver amendments to either the UC-FSNEP State Office (UC-FSNEP participants) or Network for a Healthy California Program Management team (Network Participants).

It is important to ensure sites without service are targeted and planned for delivery.
